Supportive Housing
For individuals in need of stable housing, our program offers support in finding suitable accommodations that meet their needs, preferences, and budget. We assist in navigating the housing market, understanding lease agreements, and accessing housing resources.
Eligibility
This program is designed for individuals with Apple Health (Medicaid) who experience certain behavioral or physical health issues.
To determine eligibility, individuals must meet specific criteria:
Supportive Housing
Individuals must:
Be 18 years or older
Have a history of homelessness
Have resided in a residential or nursing facility
Have experienced multiple or long-term stays in hospitals or prisons
Require significant assistance with complex health issues
Have had numerous in-home caregivers
Note: Meeting these conditions does not guarantee eligibility for FCS services. Eligibility determination is based on individual circumstances and assessment by healthcare providers or other qualified professionals.
